This form is meant to collect thoughts, reflections, and criticisms about the current state of justice, equity, diversity, and inclusion at JIRP. You have the option to remain completely anonymous; the only metadata this form will collect without your explicit knowledge is the date and time you click the submit button. Please consider entering your name and contact information, however, if you feel comfortable talking to the JEDI Steering Committee and/or JIRP Leadership about your feedback. Asking follow up questions and opening further discussion usually makes it easier for us to improve the program.

We will not reach out to you directly unless we have follow up questions. The information you submit here will go directly to the JIRP staff, and then on to the JEDI Steering Committee so they can use the feedback in building the second draft of the JEDI Action Plan. Feedback may also be shared with the JIRP Leadership.
